Please arrive 10-15 minutes prior to announced time in order to complete paperwork
No player will be permitted on field prior to completion of application

To save time for everyone, please open the info and form for Fall 2010 teams
Completion of Form
Please complete the attached form as thoroughly as possible
1 – Type or write legibly
2 – E-mail address and phone number - our only way of contacting players
Please make an extra effort to complete correctly and legibly
3 – Complete all sections except bottom
4 – Scan (if possible) player picture and e-mail to stmsoccer@aol.com as jpeg format only
5 – Picture should be wallet size photo (school picture is ideal)
Write players name on back if not scanning/emailing
6 – Avoid cut-off head shots or poor quality photo
They will not work for registration passes
7 – Grade should be grade as of FALL 2010 for Fall Try-Out
8 – Bring copy of birth certificate (New Players only)
9 – Completeness and accuracy are considered when selecting players
10- Indoor Try-outs ONLY: There is a $ 10.00 per session fee to cover indoor facility rental.
